- Fill out the Google Questionnaire in its entirety, and ensure your email address is correct. This is what we'll use to communicate with you before Skill Evaluations.
- Submit your completed form.
- UF RecSports Sport Club Waiver (Men's Lacrosse)
- Click the link above, scroll to find and click on "SPORT CLUBS".
- Click on "LACROSSE (MENS)".
- Sign in to complete the Sport Clubs Waiver for Men's Lacrosse.
- NOTE: If you have trouble completing this waiver on the computer, try completing it on your phone. If that doesn't work, be sure you've completed Item 1 (UF Men's Lacrosse Skill Evaluation Registration), and we'll sort out the waiver at a later date.
